This form is for payment of Extracurricular Activities for the MA in Education, specializing in Jewish Education, at the Melton Centre for Jewish Education. ONLY PAYMENTS AUTHORIZED BY THE MELTON CENTER OFFICE.
Payments made without authorization from the Melton Center Office will not be recognized.
