This form is regarding the payment for Extracurricular Activities for the the MA in Education, specializing in Jewish Education from the Melton Centre for Jewish Education. ONLY PAYMENTS AUTHORIZED BY THE MELTON CENTER OFFICE.
Payments made without authorization from the Melton Center Office will not be recognized.
